Request to Reissue a
Clock Hour Certificate
Our continuing ed "clock hour"
certificates of completion are handed out at the conclusion
of the class. If you have misplaced your certificate
and need a new one due to a Department of Licensing audit,
please pre-pay our records retrieval fee, and then email us
with the following information:

Your full name, and if
you used a nickname when signing in, please include that
name as well. If you have changed your name since
attending the class, please include your former name.

Your office address,
phone, and fax number.

The date of the class.

The name of the class.

The location of the
class.
-
Please tell if you would
like to receive your re-issued certificate by mail or by
fax.
